If you scheduled an in-person appointment, we ask that you bring any forms that we requested you to fill out, if you have not already sent them in to us. We also suggest bringing a voided check, should you sign up for health insurance coverage. If you are eligible and have received it, please bring your red, white, and blue Medicare card.
If you have an upcoming phone appointment, make sure to check your email the morning of your appointment for the material to follow along. If applicable, make sure to have Medicare number handy.
